2.10.x Problem mit Formular?

Hallo zusammen,

ich hab da ein Problem mit einem Formular. Ich habe auf einer Webseite insgesamt drei Formular, zwei funktionieren ganz normal, das dritte gibt mir andauernd Fehler aus.

Also, dass Formular wurde normal im Formulargenerator erstellt und in die Seite eingebunden. Wenn ich jetzt das Formular teste, kommt es erstmal zum "Invalid Request Token", die Nachricht erhalte ich aber trotzdem. Wenn ich die Tokenanzeige ausschalte, erscheint "An error occurred while executing this script!". Wenn ich jetzt die Fehlermeldung anzeige, kommt folgendes:

Kann mir jemand sagen, woran das liegt? Verstehe nicht ganz, warum die anderen zwei Formulare funktionieren und dies nicht.
