[httpbl] Http Blacklist

Andreas created a useful extension (which is pretty much as expected) to include an ip blacklist a while ago.

I thought it might be helpful to open a topic for that. Right now I'm testing it with 2.10.2 for which it seems to work. I did change the template filename to html5 and xhtml.

And I also have a question...

Someone asked for a way to test the extension. I changed the code to test if it was working (and it was), but a test mode of sorts might be nice. I'm thinking denying every 5th request as if it was a spammer, every 6th request as if it was a harvester and so on... By denying from the 5th request or so you can still see it allowing as well. Adding output to whatever location in the layout the module is in will also make clear debug/testing is enabled.
