selects menus (db) as condtionals list

Hi,
i know is posibility to use select menu as parent list with conditionalselectmenu ext.
but plugin not working for db select (for me i hope)

i got 3 level taxonomy. all levels connected and try tu do proper conditional list form and send it to catalog table.
