Building a Form

So I've built my form, and I have a few required fields.

I have checked the box that says Send form data via e-mail: Yes

When I test the form out the only information I receive is one input field.

How do I tell it to send me EVERY field that the visitor fills out? I did not check option that says skip empty fields.

In the form settings besides field "data format" you can see a small triangle.

Clicking on it shows some interesting information about format "E-mail":

Ignores all fields except email, subject, message and cc (carbon copy) and sends the form data like it had been sent from a mail client. File uploads are allowed.

So, selecting one of the other three formats will send all fields.
