Assuming you mean custom positions aside from "Left Column", "Main Column", etc?

    1. In System > Settings there is an input field for 'custom layout sections.' You can specify as many as needed.

    2. After saving your settings, when you create or edit a Page Layout, you will see a fieldset for 'Custom sections' which should list everything you entered in step 1. You can also specify where the custom sections should be inserted in your markup. I get it pretty close, then use CSS to move it if needed.

    3. After saving the Page Layout, in the 'Front end modules' section you should see your custom sections listed in the dropdown menus under the Column header.[/*:m:1w29186d]
