Tables wont update

Hi!

I am trying to update an extension. First step: Altering the database.sql by adding another Create Table. When I go to the Contao install website the "update database table" section tells me the database is up-to-date.
I also did try to create a new extension by following this tutorial:

But the same thing happens - I can not get the database to update. (I am using a mysql database and when I delete something from an existing extenion's database.sql I do get the information that the database is not up-to-date)

Any hints on what I might be doing wrong?
