Add a menu link for a form

I can see how to create a new form, but I haven't been able to figure out how to create a menu item that will display the form on the front end.

Is there a step-by-step tutorial for this, or can someone give me a clue as to how to accomplish this?

A menu item (navigation link) refers to a page.
So make a page, with an article, and add an element to the article.
Choose Form as your element type, and the palette will change to allow you to select any Forms you have made.
Make sure both the page and the article are published.
