Howto spread a form over multple pages ?

Hi All,

I need to construct a form which consist of 5 pages.
The visitor needs to fill some fields, press [next] and fill the fields on the next page (like step1, step2, etc)


how can I achieve this?

I imagine you make five forms, and place each one on the previous ones jumpTo page?
You might need to carry through hidden fields to tie them all together.

There is an extension called conditional select (not to spread it over pages though, but to allow if/then conditions in forms).
